A Successful Visit

  • Please bring your current insurance card with you for each visit.
    • Make sure Medical Associates of Boswell is listed as your PCP.  In some cases, we may not be able to see you if this is not the case.
    • Check with your insurance for coverage of any procedures, physicals or immunizations you may need.
  • We may also ask to see your drivers license or other form of identification, update demographic information, collect any copayment for your visit, and any additional payment as arranged by the billing department.
  • A parent/legal guardian must accompany a minor patient.  We must have written permission to treat a minor if parent cannot accompany the patient.  The adult accompanying the minor patient is responsible for payment of copay at the time of service.
  • Make sure we have scheduled your appointment correctly.
    • Acute problem -> sore throat, earache, fever, rash
    • Regular checkup -> reviewing any chronic problems and refill prescriptions
    • Annual Physical -> review general health, activities of daily living, scheduling health maintenance and physical
  • When scheduling your appointment, inform the receptionist if you will be bringing any forms with you.
    • Please complete your portion of the form prior to arriving at your appointment.
  • We ask that you try to arrive 10 minutes before your scheduled appointment to complete any paperwork that may be needed.
  • If you arrive 10 minutes or more after your appointment time, you may be asked to reschedule your appointment.
  • If for any reason you cannot attend your appointment, we ask that you notify our office 24 hours in advance.
